This only happens when a massive amount (more than 10?) of players get teleported to a same location (perhaps?).
Players seem to fall through moving platforms or objects after getting teleported.

Maybe it has something to do with player noblock? I'm not entirely sure.
Other than that I have no other plugin messing with collision groups. Heck, I tried disabling all plugins except a few base ones and no go.

Is there any known fix to this?

I have sv_turboyphysics disabled and have cs_enable_player_physics_box on.
The map maker designed the map for cs_enable_player_physics_box 1.

It only seems to happen when there are more than around 8 players (previously thought 10+).

Ahh are you talking about the Hexagon Heat minigame? I've ran the same map for over a year and had the same issue, it seems to be more of a map related issue than anything else, even after the latest update the author has made.

Yes, I am!
However, I don't think it's the map. I have a couple of other maps (one other being a poor PirateWars remake) that does this only when there are that many people.
It only happens when noblock is enabled, I assume.

Strange. Physics on CSGO are really wonky compared to CS:S, but the original map you listed is the only one I've ever really had that issue on, regardless of player count. I'll do a bit of digging tonight on my end to see if there are any more physics related cvars that might be causing this

One thing I can think of is fun_allinone_csgo where players rarely drop through the uplifting platform in the knife arena.
It may be happening to CS:S ported maps only?
