I'm not sure about pubfogl. but all the plugins loaded are meant for a competitive gameplay (confogl). which removes medkits in start of map. I hope someone else can help you here...

I've tried and play with the settings but I just cannot get more than 8 human players in the game. It will always end up with 4 humans and 4 bots on each side. Can someone please tell me what I'm doing wrong? I'm tearing my hair out here.

You need an extra plugin to use the unlocked slots. I used
Left12Dead but do note that you should download the sp file and change the limits. Because I've gotten more than listed bots and suddenly it becomes like a party.
