I have taken it upon myself to make changes to the code to add a combat shotty in the code (and have it working) but now I am noticing that the first post of this thread uses an older code base without the minigun, and the newer code (with the Minigun) that is compiled on post 426 offers no sp source code. I tried cannibalizing naka's code to add the minigun but it won't spawn for me. Granted, I could be doing something wrong because I am not a great programmer by any means.
Regardless, here is the source and smx for the combat shotgun integration. Also added Lounger's suggestion of adding a 25 character limit to weapon spawning and I increased menu fadeout time to 25 seconds. I did not include code for the hidden/censored weapons.
Rename the plugin to remove the _test in the filename.
Does anyone out there have the source for the latest version from post 426 of this thread?

It says "Failed to compile plugin! Please contact the author." when trying to download.

Shame how people tend to forget about great stuff like this.

Also, sorry for the double post, but this plugin crashes my game after 2 minutes, and I'm pretty sure it's this plugin. The above all4dead2_test.smx exhibits the same behaviour.

I think it happens every time a witch is spawned. Perhaps this could be fixed?

Does anyone know how to raise the cap of special infected that you can spawn? I can't get more than a few on coop.

After abandoning this for awhile I came back for a second look. Using my small addons and portions of Naka's I am still trying to figure out how to spawn the minigun in the menu but at least I am able to bind a key manually in the console with
bind <insert name of key> "a4d_smg"
this spawns the minigun at wherever your crosshair is (as an in-game admin) Using the in game talk/say function you can also type !a4d_smg
bind <insert name of key> "a4d_rmg"
this removes whatever minigun is in your crosshair (as an in-game admin) Using the in game talk/say function you can also type !a4d_rmg

Because this plugin uses SDK hooks, it probably won't compile in the browser compiler.
Inside the sourcemod folder, there is a scripting folder. In Windows, just drag and drop the .sp file on the spcomp.exe.

After the 2nd July update the "Spawn Special Infected" menu no longer functions correctly.
The only specials that spawn at the cursor are Tanks and witches, None of the other specials do.

Thanks dude it works...but i'm puzzled I also changed the z_spawn to z_spawn_old in the script
but mine didn't work. which means i'm a muppet, I uploaded the old smx or there was more to it than simply changing the z_spawn cmd.

I'll take the latter being a muppet doesn't sound good to me

I am now getting this error when starting the server...

Unable to load plugin "all4dead2.smx": Required extension "SDKHooks" file("sdkhooks.ext") not running

SDKHooks 2.2 is installed, so any ideas why this is?
