OMG what a stupid mistake -_-

Last line in server.cfg was "map de_dust".
Seems that this confused the server from time to time while map changing.

Removed it and now the server is running...

Yes the server is up to date. Its a fresh clean installation with all updates and latest plugin/ mod versions.
