i can't go to my modem's site also
i doesn't comes
but i want to do port forward
but can't find the name

Modem or router? If it's only a modem you don't need to forward any ports. If it is a router instead simply do this:
-klick on "start"--> "execute" (or however it is called in your language)---> enter "cmd"
-In the upcoming window enter "ipconfig/all"
-copy or remember the ip listed under "DNS-server" and enter it into your web-browsers window.
Voila, you found your routers start page were you definate will find the name and, if you login the modelnumber and revision.
