Happened to me once yesterday. I believe it happened to everyone actually. Simple server restart fixes it.

I've only encountered the issue once though on my server.

Yep, this is super annoying.
Is sourcemod causing this or CS:GO?

Just happened again. It seems as when steam goes down (ie. Community, Friends, etc). That it also causes the servers to create this issue?

Hmmm, steam was up for me when this started happening.
It might be the steams cloud servers? Who knows. :/

We'll were you able to catch it exactly when it happened? I was testing and as soon as steam went down I wasn't able to reconnect to the server. Steam was back up around a minute later, but I had to restart the server to get into it.

Who knows though could have just been a random occurrence, lets just hope they fix whatever is causing it.

It seems to be the steam community/friends. I tested it, as soon as I lost connection to friends list. I would start to get failed sessions.

This is a mapgroup error, I just created a tutorial on how to prevent this from happening.

I don't think it's the map group join problem in your tutorial. This is talking about servers that are running but randomly begin to block connecting and require a restart. Strangely this happened on one of our servers today. Maybe it's steam related. I notice in console some Meta Mod references. I asked if it might be a MM bug here:
