This issue has plagued my servers for a while, I'll give this a go and return some feedback!

Unfortunately I had this happening on one of our training servers which didn't have any plugins besides SourceMod itself.

And this also happens on pure vanilla servers as I stated in my description.
I verified this with a friend later on by hosting a pure listenserver (directly via client, no servers involved).

Last time, i found this bug on Valve official ds too. So this is not SourceMod plugins issue.
