Can I use an attribute mod on TF2Items or do I have to use one of the Custom Weapons mods?

A plugin that adds additional attributes that the vanilla item schema doesn't offer.

TF2Items includes the source code for a plugin called tf2items_manager that can add weapon attributes to existing items, you need to compile it yourself.

I meant attributes that aren't in the game, not weapons with different attributes.

No, those "attributes" are just hooks modifying state, they are not integrated into the game and just act as a configuration method to choose what to enable for the plugin that implements them.
