Still getting the same issues, even with both of these suggestions being implemented. I also turned down sv_timeout to 60 to no avail.

Stop copy past stuff... he already use a steamcmd server... and he is using latest amx and metamod...

Well, this problem happened to me when server was not updated to the last version, i saw ways and it was fixed..
Look, try this cvars:
sv_proxies 0
sv_alltalk 0
sv_voiceenable 0
sv_timeout 25
mp_consistency 0

Sometime crash on 32/32 servers if on and if timeout was high..
Also motd.txt should be short after i did all of that ( after i searched here ) my problem was fixed, i hope if this help you, don't use plugins just try to use cvars and fix motd.
