The server is playing COOP on map l4d_hospital01_appartment (easy mode)

Stats are disabled in Easy.

Also, it looks like you have something wrong with your database. Either the info is wrong, you didn't restart the server after updating your database.cfg, or your host doesn't allow external MySQL connections.

look inside /leaf4dead/cfg/sourcemod/l4d_stats.cfg
for the line :
sm_l4dstats_minhumans "2"

change the "2" in "1" for stats enabled when only 1 player on server ;) it works on my config ;)

I got a small issue, with the plugin. Posting to see if anyone has any ideas, I am out.

I run 3 L4D servers, and the website of off the same box. 1 server of each gametype. I installed the stats plugin a few days ago on the Coop server, and it worked great. The other 2 servers I added today, and the plugin functions, but is not logging stats to the SQL database. I have them all setup to the same database, all passwords and settings have been triple checked. I even gave each server their own SQL user after I found them not working while using the same user. Still no luck.

I can't find any reason that this setup is not working. Does each server need its own database? I can't see anything in the code that would require this.

No, each server does not require it's own game type. Survival and Versus are not supported yet. They are coming soon. Survival is currently in-testing, Versus will follow shortly after that.

Sorry if this has been mentioned, but I'm noticing a few issues with the scoring and it may relate to another problem I'm having. It seems certain actions are appear twice for each occurrence, such as FF and a group completely dying.
