[TF2] Infrequent Crash on Map Change

This is a problem that has been plaguing my server for quite a while now. Whenever the server changes maps, usually due to RTV, it will just crash before it switches, leaving users stuck on the Server is changing levels screen with 3 bars. This doesn't happen every day and it isn't always caused by RTV. There are also no errors in any of the logs around the time it happens. I recently installed Accelerator on my server hoping that it would give me more insight on the problem. Good news is that it did, bad news is that I don't understand anything it shows.

Edit2: Removed some maps that seemed to have caused the crash. Server is also now on a Linux machine. Will wait for more crashes.

Does it crash only when changing to a specific map(s)?

It doesn't crash when changing to a certain map but it does seem to crash more frequently when changing from certain maps.

check if the map you want to change have on your /tf/maps and on your FastDL website(if have)

recommend you to use downloader plugin. easy for newbies and simple to use

After moving to a new, Linux server, the crash still happens albeit much less frequently. It doesn't seem to have anything to do with the map it changes to or the map it is changing from. It just seems to happen at random when changing maps.
