Yes, I only support Linux installs, but source code is there for anyone who is willing to compile it on Windows.

Probably too late to mean anything now, but all that has been done to the extension is have some forwards added and made to work with latest updates. All original functionality remains and can be found in this thread on the original post.

The Server is crashed, when used console command "changelevel" or level change when map end.

What makes you think that its Downtown crashing the server?

Because when I install left 4 downtown, the server crashed, without extension server successful loaded

Error logs from SourceMod is missing. Only print default message where need -debug in command line start

Do you have Accelerator extension installed? If yes, then can you link the crash log from that.
