My server apparently was down all of yesterday and I had no idea. This was the reason when I checked my server console through LGSM:
That was spammed many, many times. I checked the folder listed, and there was a .dmp file in it, but its ownership was set at 0400 for some odd reason. So I CHMOD'd the file to 0777 and rebooted the server. The minidump file disappeared. This leads me to believe that if the server tries to write another minidump file, this will happen again and my server will suffer from major downtime as a result. What the heck is going on here, and how can I stop it?
Also noticed some odd error lines in my log file, but they're unrelated so I've spoiler'd them
A ton of these out-of-range value errors. Like, literally a ton. After bookmarking every datatable warning line in my log file from the 2nd, copying all bookmarked lines and pasting them into a new document, I have a total of 13,986 lines. Some are for the ammopack, some are for a player, etc.

Some of these weird entity change errors too. I'm guessing these are a result of a plugin that messes with ammo, i.e. Infinite Ammo. However, there are some that I don't think are a result of ammo plugins, like the ammopack and player lines. I think the ammopack line may be a result of whatever Scout weapon causes you to drop an ammo on death. That may also be the reason for the out-of-range warning for the item_ammopack_medium entity listed in the previous code box.

Finally, and most unrelated-ly, I discovered that apparently a few people have tried to hack into my server's RCON, judging by these 4 entries I found. I haven't checked other logs so I'm not sure for how long this has been going on.
