"Player not on the server anymore" Error

Installed Sourcebans 1.4.11 and even updated it from https://github.com/GameConnect/sourcebansv1, and I still get the error trying to kick/ban players from my sourcebans webpage. Any help is appreciated.

Could you please elaborate with some more information?

- What game are you running?
- What metamod version are you running?
- What sourcemod version are you running?
..etc?
