I thought this happens if someone is using a hacked (pirated) version of CS?

Theese guys don't play. It happens when the server thinks, a client is connecting but did not get the timeout yet

our hoster automatical puts STEAM_ID_PENDING in banned.cfg - wont this fix the problem?

No. Everyone has STEAM_ID_PENDING upon connect and if it does not change, they are not checked against the banlist

What about HLDS's stock, Invalid Steam ID Ticket? Does that have anything to do with this at all?

I'm a little confused as I've never seen a user with a STEAM_ID_PENDING auth ID. Would such a player actually be active in a game? Is this somewhat common or pretty rare?

Common? Well...
