[TF2] Server Crash

I've been trying to solve a crash that happens randomly on my server. Tried using different sourcemod versions, removing plugins and nothing worked. So I'm posting here for help.
I noticed the server and a few clients crashing when a player disconnect and sometimes and changing map.
Accelerator Crash ID: LBF4-RQLR-O7NQ and IHDC-FPYC-FR6F (Latest)
It's a Linux server.

Try SteamCMD, update your server with "validate" parameter

Sorry for the delay. I can't validate my server (I don't have root access). I did reinstall the server from the control panel. I've also spoken with the host and the server files are validated every 20 minutes. Even after the reinstall the server is still crashing.
Edit: More 2 unique crashes. HAUL-DOI6-CTBE and TIHB-SWP7-XFRO
