I'm having the same issue so it's prolly not something on your end. After the l4d2 update I get long periods where I can't ping the server. Not sure what it is yet, I have another l4d2 server that doesn't have the issue afaik.

I'll research after work and report back unless someone else gets it

can you send me a list of extensions and plugins? or your server ip?

Server lags even without plugins, probably year ago (maybe less, maybe longer), it was working without any problems, maybe I should reinstall? ah ok, we have different issues then. In your case jst run a repair or reinstall.

I can add that I run tf2 server with about 40 plugins without any lags...

Reinstall did nothing, still lag attack.
