It's been successfully reproduced by people (not Valve) on TF2 servers running no plugins, Valve are having trouble reproducing it internally which is what's delaying a fix.

This is not true as death is not necessary.
Also this happens with every non-stock flamethrower.

Tests on tr_walkway_rc2 (cmd).
The problem also is on the servers of public.(hosting)
Main and use airblast type weapons The Degreaser.
The problem is observed on all servers.(hosting and cmd)

Unless I've accidentally missed something, yes. The files I provided can be used to quickly start a db server. I only included the db files so tweaking cvars of tf2 itself and other plugins is your choice there.

If you run into an issue, feel free to post and I'll check if I missed anything but that should be everything.
