Do you have uploadet the .inc files too from r149 ?

addons/sourcemod/scripting/include/sb_bans.inc

Just check at first if you upload the sb_bans.inc

If it's not work after uploading the sb_bans.inc you can try to

delete the sb_bans.sp and sb_bans.smx

Upload the sb_bans.sp again to /addons/sourcemod/scripts/ and compile* it by your self and try again.

*to compile run the compile.sh -> wich you found in your /addons/sourcemod/scripts/ directory.
after you run the compile.sh your will found the compiled script at .../scripts/compiled/sb_bans.smx.

Put the ../scripts/compiled/sb_bans.smx and move it to ../sourcemod/plugins/*

restart your server and look whats happen.

If it not will work type this into your GS console and show us the result

sm plugins list; sm exts list; meta list
