Login to steam, then join a server.

Edit:
The images you see are people who are login to their steam accounts, that's their steam avatars that you are seeing.

You can't, it's client side.

Then you can't do anything. It's entirely handled by Steam. If you want to change your image, then change your profile image in Steam.
