spawn ghost tank? maybe change spawn tank plugin.
or try remove infecd fix plugin.

I use the multitanks_b-l4d2.smx plugins with the following configuration multitanks_count_coop_regular "2" and sometimes that error occurs with the tank. The other complement that I use for special infected is l4dinfectedbots.smx

Find the error plugin and ask the plugin author.
