This may be of use to you, it was the last entry in the latest error log:

Code:
L 11/09/2010 - 20:14:19: [SM] Native "Format" reported: Client index 2 is invalid
L 11/09/2010 - 20:14:19: [SM] Displaying call stack trace for plugin "fortwars.smx":
L 11/09/2010 - 20:14:19: [SM]   [0]  Line 641, D:\PawnStudio\compiler\fortwars.sp::Timer_Tick()
EDIT: Just updated everything to the latest versions, still crashes. The seg fault line has "$HL_CMD" at the end... it must be something between sourcemod, metamod, and/or the hl2 binary that's invalid that's causing a seg fault.

Upload the most recent file in addons/sourcemod/logs that starts with "errors_".

I went to your server, and I can crash it as you described, but I can't crash my server. My server runs on windows, I tested on a linux server too and it doesn't crash.
Same issue here. Here is the error from the logs

Your issue is different, it seems you don't have TF2 Tools installed (It is not finding a function from it). That's weird because it comes with sourcemod.

Whip, that's just a simple error, I'm gonna fix that, but it is not causing crashes. Upload the whole file.

Something I just realized, my latest error_* log is dated the 9th. I've been testing this stuff every day, so since there's no log for yesterday or today, the error that causes a seg fault isn't in there. Anyway, if you really want it, (it's just the same message I already posted repeated over and over) I attatched it. I also updated to 4.0 and the bug's still there.

The update was for some mappers that requested a way to control which team can build.

Did you try to reproduce the bug with FortWars uninstalled? Do you have another server to test it? Anyway, add me on steam: M2828.

Wow, great idea! It still crashes on fortwars maps with the plugin disabled! However, it (switching classes outside the spawn zone) still works just fine on valve maps. The plot thickens...

Well, It seems that it is a map issue, that is far more complicated to find, but... Why doesn't it crash on my server? :O
Btw, you didn't add me yet.

Good news, everyone! We've managed to squash the bug! It turns out there was no spectator camera on the fortwars maps. So, if only one player was playing and died, they had no camera to spectate from and the server crashed. One little check and it works now.

Anyway, here's another bug:
Code:
L 11/11/2010 - 15:59:05: [SM] Native "GetEntPropVector" reported: Property "m_vecPosition1" not found (entity 68/trigger_capture_area)
L 11/11/2010 - 15:59:05: [SM] Displaying call stack trace for plugin "fortwars.smx":
L 11/11/2010 - 15:59:05: [SM]   [0]  Line 2046, D:\PawnStudio\compiler\fortwars.sp::FindObjectives()
L 11/11/2010 - 15:59:05: [SM]   [1]  Line 786, D:\PawnStudio\compiler\fortwars.sp::teamplay_round_start()

That's the famous no spectator camera bug.

My fw_ maps don't have cameras, so, if a player dies and there is no one to spectate, then it crashes.

Fixed all bugs reported here.
