When trying to load the ext, I am getting:

Engine: CS:S
OS: CentOS

Redownload the newest version of the extension and try that out.

That is the newest version he's trying. Make sure your server is up to date.

He's on CS:S which doesn't have the libvstdlib_srv.so that you linked against when compiling for TF2/DODS/HL2DM.

You need sourcemod 1.5 for it to work on CS:S

SendProxy 1.3 is working fine.
