^^ what asherkin said. Attached is the voice.cpp I'm using now minus the logging calls for troubleshooting. No binary is included. I believe the problem was triggered whenever a player disconnected and a plugin author did not use a 1:1 correspondence between the listener and receiver with the overrides. In my own I allow CTs->Ts but not Ts->CTs for the first 30 seconds, which eventually will cause the problems to occur. A hypothetical situation causing this to happen:
