The title says it all.

I'm wondering how can I attach a custom model to a grenade or weapon e.g tree to the item through sourcemod

teleport the entity/model created and parent it to the entity

A little hint of good practice here, do not set targetname for parenting, use !activator in AcceptEntityInput.
