Server Crashes upon Death

Well i am having a problem with the server that everytime when a death is caused from any plugin (nukem, gas, super admin...) the whole server crashes and shuts down

how can i fix this?

It means the map doesnt have an entity that its supposed to.

i tried that plugin but it did not work

Ah, then its probably not death, its suicide which I believe is what those plugins do. This was a problem with Sourcemod, which im not sure if it was ever fixed or not. I would try upgrading to the latest SM (snapshot) and see if that fixes it. If not, its up to the plugin authors to use alternative means.
