[l4d2] nextmap.smx error when specific user is in game on custom server

I am have issues loading the next map in L4D2 on custom campaign in when multiple people are on our custom server and more frequently when on specific user is in game. the error log is attached. what can i do to fix this?

As error said, the plugin is incompatible with L4D. You have to remove it.
