mapcycle.txt syntax/formatting?

My current mapcycle.txt looks like that.
There's a problem though, it keeps messing up by itself, some maps will randomly get the cycle "stuck", for example we will be playing dust2 and then all of a sudden the map cycle is screwed up and it keeps setting dust2 as the next map.
I would like to know:

1) Do workshop maps have to be set like that (including the folder in which they are included) or should I just set the map name?

2) Why in the world is this happening? It's driving me crazy.

Any help will be really appreciated.

The goal is to have this fixed mapcycle, no map votes, nothing.

EDIT: So I had this issue again, map was stuck on mirage. Reloading the plugin nextmap fixed the issue, so it's something to do with that plugin.

You need to delete the slash in front of "workshop". Don't know if that's breaking it or not

Yes thanks that was causing the server to get stuck, I'm trying to see if that fixed all the issues.
