I ran into this myself while setting up... the UDP ports aren't fully open by you're host yet which is why you are still getting that one error. Have you're host open those up for each IP# and each game port

I don't know..sounds like you're webhost is being a pain in the butt and not wanting to do what you ask. They shouldn't care what the target server is doing or isn't doing...you ask that those ports be open and they open them - if they don't find another webhost that's all I can tell ya.. I can make a suggestion for what I use and it's only about $180 for 2 years which isn't bad and they opened what I needed within an hour of doing a ticket.

Yes they don't want to give any attention for this ...

I would like to know which one you use because I'm beginning to study the possibility of moving all my hosting services them.

Hi,

If you unban someone when will this user be able to join the server again ?
I tested it now and i could only join if the server was restarted.
on map change I could not join, still baned.

You should the configs/sourcebans/sourcebans.cfg. There are interval seconds that server refresh the ban list.
