I HIGHLY suggest going back and removing that tf_server_identity_token value from your post.

Anyway... shortly after Valve introduced sv_setsteamaccount they changed when
tf_server_identity_account_id and tf_server_identity_token were checked.

Now, they need to be put in the server startup command line, autoexec.cfg, or some other configuration file that runs on server start (for multi-server configurations).
