your setup looks good.
failed to join session known bug, we all wait fix from valve.
this error happen when server lost steam connections.

If anyone is on the mailing list for CS:GO Valve, this is currently a hot topic issue. As it's been a problem since the game was released. I would suggest giving some insight if you know possible remedies. This is only an issue for public owned servers, not an issue for Valve servers.
