I've been trying to get to the bottom of this error for months, and still can't even get a clue to what it is! Wondering if anyone could help.

The crash is definitely starting when map changes, and it hangs at ---- Host_Changelevel ----.
I know it isn't related to specific gamemods since it happens to servers without them. It just happens randomly after the server has been running for a while and usually happens once a day. It happens more often to servers that changes map frequently, rather than just once a day for 24 hour maps.
I thought it was mysql related issues, but all my plugins are threaded. So I'm not sure it may be plugin related and it may be either due to hardware issues, Debian 8, or the script used to start the gameserver?
My bet is on it's related to Debian dependencies as it seems like glibc is related to libc-2.19.so and I remember the OS version is a problem with this.

Is there any proper way to go about finding what may be causing this or am I stuck with just guessing and trial and error? I tried pinpointing a plugin but uninstalling but no luck.
