I must say that this plugin is indeed quite unmaintained (but should still work), and I have unfortunately don't have much time to do so, but feel free to make pull requests on github for updates so I can update the offsets/signatures and edit my post.

Fixed crashes in TF2 by increasing tf offsets by 1. This might help with other games.

I went ahead and verified that the offsets linked from GitHub and provided by Crasher above are still correct ever since the TLS update first came out.
