I wasn't able to reproduce this behavior, tested on linux windows on all sourcemod versions.

Isn't there a specific error why the extension fails in the log somewhere ?

He didn't come to me yet.

I think sourcemod only tries once to load the extension and does not try again if it failed once.

Unfortunately I don't know why you can load it without error and it still says module not found, that's weird.
