This plugin saves up to 100 disconnections, which can be modified by this cvar:

ban_disconnected_max_save

This plugin uses new syntax and uses the reasons that appear here for you to easily edit: addons/sourcemod/configs/banreasons.txt

The plugin integrates with my SQLite bans plugin, making disconnected bans become IP bans as well, also adds the nickname for both disconnected bans and sm_addban.

This plugin also has more time options for bans, and updater support will ensure any improvements I make to the plugin can be added to your server.
