SourceBans not working, can't figure it out

I need some help figuring out sourcebans. It keeps throwing up errors and I dont know why

Please paste logging information into either [CODE] or [PHP] tags on the forum, it keeps the formatting and doesn't add smileys, etc.

That pretty much explains the solution to your problem. Follow the documentation and add the appropriate database information to your databases.cfg.
