I have 1.4.8 but thats not to say they haven't found a vulnerability in that too.

They accessed a cfg file called /cfg/game.cfg. Added allot of blank lines then at the end added steam id of admins and banned them in cfg hence not being able to remove the bans for a while.
Its very possible they got these id's from sourcemod. I will know next time as server logging on the box is running 24/7 and every connection to the servers are logged.

Are you sure that there is not a plugin that has been added that is doing it. Try disabling all plugins and then working from there. It is also hard to solve this if you do not have full control over your server as you may need to check the directories directly to ensure that they are not read only ect.
