Figured I'd share my solution here.

Running a CS:CO server, it was unable to boot with Metamod 1.11.0-git998 and SM 1.9 build 6080, even with no 3rd party plugins/extensions. I used MM:S 1.10.7-git956 and it now boots and works perfectly.

That'll be because 1086 is the first 1.11 build after the fixes were made, and 1.11.0-git998 is from September 2015.

It looks like the snapshots page is being ordered incorrectly for MM:S (the snapshots are being ordered lexicographically rater than numerically).

Yoink, didn't even consider looking for 1000+. Whomever has access might want to change the order to numerically if possible, because I'm sure I'm not the only one who overlooked it.

My discovery would go against what psychonic said about 1.9 requiring MM:S 1.11 from here on forward (can't find his post), but I'm assuming that was because he probably thought 1.10 wouldn't need to be updated anymore, considering its last update was January 2015.
