There is a problem with this plug-in. The default path is timeleft/fvox. If you change it in the cfg file and reboot the server, you get an error saying that the path timeleft/fvox is not found. But on the next map, the sounds will work. I found that I had to change the default path in the sp file and also change it in the cfg file for the sounds to play on server restart.

Sorry, but I can't replicate this error.
Be sure to set sm_timeleftvoice_path to something like "timeleft/unreal" and NOT "sound/timeleft/unreal".

If you still get that error, let me know what game (css, hl2dm) and sounds you try to use.

I am running a Goldeneye: Source server.

In my cfg file I have:
sm_timeleftvoice_path "timeleft/unreal"

I start my server and look at which plugins are installed using the console. I do not see timeleft voice. I check my logs folder in source mod, and I find an error log.

Your mod is looking for the fvox sounds when I have told it in the cfg file to look for the unreal. On the next map, I look at which plugins are installed and I see TimeLeft Voice. The sounds works. The only way I could get the sounds to work when the server first started was by editing this line:

#define DEF_WITHIN_SOUND_FOLDER "timeleft/fvox" --> #define DEF_WITHIN_SOUND_FOLDER "timeleft/unreal"

Also your version should be updated:

#define PLUGIN_VERSION "1.4.30" --> #define PLUGIN_VERSION "1.5.30"

I kinda found the bug, but its still not working as expected.

Give me some time I gonna rewrite it from scratch!
