Your best bet is to enable the voteban system. It's not that effective since they can change names constantly but It's your best bet so you don't sit on your server(s) 24/7. You could also setup a plugin where people can't change their names often to fix that as well.

SMAC already kicks for name change spam.

That doesn't block out hackers that have a lot of money which most do. On top of what you said, enabling Voteban is another viable option and just disabling name changes on the server to every 5 minutes or so would help.
