The problem appears to lie in the "revive_success" event.
The following version has debug code at each point in the revive event where a player would receive dual pistols. I think I worked out which one it is, but I want to get the debug info beforehand so I can fix the problem more easily.
"%N was revived." will appear every time the revive_success event gets called. If its not too much trouble can you keep track of what points it gets called at (i.e. if it gets called when someone gets picked up from hanging off a ledge).
"%N had his Dual Pistols returned." should appear only in mode 3-4 when a player is revived and was using pistols before getting incapped. This is the only time someone should receive pistols.

The following chat lines only appear when the plugin is trying to cover for an detected problem.
"%N was given Dual Pistols as backup." is displayed in mode 0-2 or when a player was using melee before getting incapped but they don't have a weapon at the moment.

"%N was given Dual Pistols because there was no valid weapon for replacement.". This is the one to watch out for, I suspect this is the part of code that is causing your issues. Its caused by the revive code getting called without the 'about to be incapped' code getting called beforehand to determine what weapon the player is holding, so it just gives them the default Dual Pistols.

Its official, I'm an idiot. I was just looking at events and found that I had COMPLETELY missed the ledge hang boolean on the revive_success event.

I don't think so. The golf club should have the same weapon name as the rest of the melee weapons

using option 4:
if you have a single pistol, when you get revived, you have duals.

That has been mentioned several times before. Its the intended behaviour of the plugin.

Ok, I'm back everyone. And I got a new bug to fix for Incap Magnum.
You don't get your old weapon back if you get defibbed after you get incapped and died. I'll probably add translations and some of the other requested features.
