I'm having a problem where occasionally my server will crash with the faulty module being dbi.sqlite.ext.dll. My server is a Windows Server 2012 R2 if that helps. I'm not sure where to find dump files or w/e is needed to help debug this. Any help would be appreciated.

Using the latest dev snapshot 1.8.0-git5555 srcds.exe crashes with the faulty module dbi.sqlite.ext.dll right on start up. I'll give the stable branch 1.7.3-git5233 a shot now.

EDIT: Same exact crash on start up with latest stable.

Then you should be using the accurate data from Accelerator and ignoring the messages from Windows about dbi.sqlite.ext.dll.
