I solved it. I found out that for some reason my game sets paged memory in video settings to High for some reason... I don't how it went from Low to High automatically 2 days ago... Likely thought it has to do with my server.

-----------------------------

So yesterday my game and the 8 player server was fine with sourcemod plugins, until last night i disabled the takeover command for ABM in the config because of an exploit and my game crashes once during late transition of the maps. Today it becomes worse, I lost some players due to my game crashing during early transition of the maps everytime. I never touch my server and its configs this day and i thought it has to do with takeover command, so i enabled that in the config and the crash still appears.

I checked my console.log file and couldn't find the source of the problem. Even checked the sourcemod error log but these were actually Improved Automatic Campaign Switcher errors and stuffs that weren't related to it because they were before the crash occurred. I also restarted L4D2 and switched to Infected Bots Control, Multislots and CreateSurvivorBot instead of ABM, crash still occurs...

UPD: Earlier ago, i forgot i had two transition plugins: Transition Info Fix and Transition Restore Fix. I tried removing them one by one (TRF replaced by forked Save Weapon plugin during that time) and refreshed the plugins yet im still crashing.

Here's all the plugins i installed. I was still able to reproduce my issue after deleting some plugins like Witch fixes, item footlocker, etc. I couldn't post the plugins here because the forum thinks i used 100+ images here: https://pastebin.com/AVANRda8

What are the meaning of terms "early" and "late" transition and what are differences?
Can you provide crash report of accelerator?

Early transition is around "Killed the most infected" screen, and late transition is around "Did the most damage to the tank/witch" screen. I'll post a crash report tomorrow since I am typing this on my phone. But wdym by accelerator?

The accel log only shows the dump info including the token and the crash ID unfortunately, but my dump is currently processing in Throttle if its possible.

UPD: Its now processed.

You'll better ensure it is a correct crash dump by catching one more and compare.
Also, on un-authorized browser there are not much info by the link you provided.
To be able do something, I need copy-paste of first page available when you click button "View Raw".
