Google is your friend.

Q: My game crashed with this error message: "S_Findname: Out of sfx_t"

A: When Half-Life reaches 512 cached sounds it causes the engine to stop. On dedicated servers, you can rejoin after the crash, but if you are hosting a non-dedicated server it will dump all the clients. If maps are designed correctly, this crash should never occur... the problem is that the number of monster types used in a map needs to be kept low. The other problem is that a few maps do use too many sounds (it's hard to judge, I admit).

I am fairly sure I was the first to say, "Google is my friend, it should be yours too!"...

So you should give me karma for adopting your phrase, and passing it on to future generations.
