All the approvers are supposed to be trustable users. I dont see any reason for use a separated moderator. It would take more time to update, and the moderator can get escape some plugin. Not if it is updated by the approver.

File. In this case file means to submit or create (a bug report).

It has nothing to do with finding or reporting bugs. They are simply using the bugzilla system to track approver/moderator applications. It was a quick and simple solution that didn't require additional software to manage.
