Hmm im not sure but heres a few things i would make sure.
1. Install the stable version - Download stable-Version | SourceCode
2. Maybe allow external connections to your database?
3. Make sure the info on database.cfg is correct
4. Look at the error logs from sourcemod
5. have the latest verison of phpmyadmin/mysql?

i try it on my test server and karma working and on main still don't work kappa

Server still not allowing teams/roles to be assigned. Time runs all the way down and Detectives get an automatic win.

Clean install on Windows machine, plugin/mod starts up fine and ttt.cfg is executed properly. Only thing missing is giving roles to individual players.

Edit : Only errors I'm getting right now

Which version you using ? Stable,Release or Git ?

I see some TTT maps dont have any weapons spawned so i've been looking and i found this.

It works 100% and you can spawn any guys any where for whichever maps you'd like.
Some of the maps i found without guns spawned: ttt_subway, ttt_stormy_days_slayers

Oh ok, sweet!

Also i encountered a few bugs:
1. You dont die or get injured from fall damage.
2. You can spam the chat with things such as (Player: Chevalier, Karma 99, Credits: 800) This triggers every time you knife or shoot anyone or anything.

Which version? Which translation phrase? I need more information... and don't like "I've found a bug, but I'll don't give more information".
