Yes, because plugin_cfg() is called at map start ( "Called when all plugins went through plugin_init()" ), but it could init forwards and settings in plugin_precache.

Done. And yes, plugins do not need plugin_init to work. In theory, you just need a forward called early enough to initialize all your hooks.
