Yes I get this:

[SourceBans] You can not add a ban form in the server, visit (website) to add a ban.

But it works fine with sm_ban. I just need sm_banip as well.

Turns out the problem was a version inconsistency between the server and the website. Sorry, false alarm.

Manual Prerequisites Webserver PHP 5 required At least MySQL 4.1

But if i try to install SB it says i need MySQL 5.0.
My Hoster dont want update at this time. What can i do ?

With L4D supporting multiple instances from one installation how would you configure the sourcebans back end to support that?

Since each server needs its own sourcebans 'id'

Make the following changes to the config and test. It should work as since the +ip and -port are on the command line. This is no different from other games, SRCDS has always supported multiple instances from one installation. But I thought MetaMod:Source didn't run on L4D?

I have 4 servers 3 of which are HL2DM servers and I have been enjoying Sourcebans for a while. I just added a HL2CTF server, set it up the same as the other 3, changed to the correct server ID in sourcebans.cfg, made sure mysql was connecting properly, however in Sourcebans I still get this "Error connecting (72.5.86.37:27016)" Anyone got a suggestion?

One bit of info, the 3 working servers are linux the new CTF server is windows.

The gameserver is likely blocking the query or your web server is blocking it, either way something is blocking it like a firewall.

Yeah, that's what I though, srcds has had multiple instances for as long as I can remember.

And yes you are totally correct, MetaMod:Source does not work on L4D.
