Tried to join again here and was vac banned. His Steam ID page now shows bans on record. Whereas before it did not.

Did you ever get it working? It looks like it failed to connect to the database for some reason. Were there any other errors in the log before that one?

The plugin seems to work, but the error is a bit annoying. Problems started 11/28/2009. No manual changes was done on the server at the time.
Any clue? Do you need anything more to narrow it down?

Status is banned now so maybe that just happens later?

What do you mean? The bans are done using the sm_ban command, so it will go to SourceBans if it is installed.

rcon commands in a .cfg file?

When server have tv_enable 1 (SourceTV)
Server create "bot" what is SourceTV itself
When launch server it give those error but not after that.
vacbans.smx try find sourceTV vac-status ?

When server tv_enable 0, it not given those errors upon startup.

I try also with bots, adding bots beginning when server startup autoexec.cfg (no SourceTV, tv_enable 0)
bot_join_after_player 0
bot_quota 5
vacbans.smx give those same errors when launch server.
