Ok this what happens sometimes when round starts random people gets slayed 2 times for nothing and says this :

Player slayed for playing the same round twice.
Not sure what mod makes this happens, but maybe someone know cvar that disables this?

Are you using mani too? that sounds like a feature mani has.

There's a setting for in in the mani_server.cfg

Yeah, I was about to say none of those plugins you have loaded do that so it had to have been something else. Mani seems to be the source of a lot of problems on these forums. :\
